Established for over 40 years, we are one of the largest independent drug agencies in the North West.Shared Care Worker – Ref SCW005Primary Care Team - Liverpool – £21,412 - £23,749 pa (SCP 26 - 29) 35 hrs pwWorking in partnership with GP’s and carrying a caseload of opiate using clients, you will plan and manage effective packages of care, undertake assessments, formulate care plans and refer clients to other appropriate services to support and assist them in making positive progress towards achieving stable lives.You should have worked within the drugs field and have experience of assessment and case management.Applications for all this post must be submitted before midday on Friday 20 June 2008.To receive an application form and further details, send a large s.a.e.
