We are currently recruiting for an experienced Qualified Social Worker to work for our client, a Central London Borough Council. The successful candidate will be based within a hospital setting, so 2 years post qualifying experience is essential. Main Job Purpose:To deliver exceptional social care to adult service users and their carers within a defined budget, enabling them to take their own decisionsThe client may consider offering a higher pay rate providing locums have worked for an adults hospital team & have 2 years plus post qualifying. The Team Manager however, is not looking for SP or Manager level so only experienced QSW's please.The successful candidate will be based in the Information and Access, Assessment and Care Management, Reviews team at the hospital. Candidate must have understanding in theory and practice FACS criteria Community care Act Discharge act, (understanding sect 2 and 5 as well as timelines, reimbursement, charges and timely and safe discharge) Adult protection New mental health act Mental capacity Direct payments Locum needs to be able to work under pressure as hospital work is very demanding and there is high turnover of clientsIdeally we would need somebody who has at least 2 years experience of working in the fast paced hospital teams but someone with over 1 years experience may be considered. Experience in assessing clients over 18 in hospital settings, as well as putting in place care packages that reflect client needs and eligibility criteria The successful QSW will be expected to work as a duty SW a few times per months, part of their duty will be to take initial assessment and screen the referrals as well as manage and answer any questions and queries The Qualified Social Worker will be expected to manage a case load of complex and straight forward hospital cases The person must have the ability to respond to workloads that relate to hospital discharge time frames Experience in performing hospital reviews in community and residential settings and also in attending Multi Disciplinary and the Discharge Planning process Negotiating appropriate discharge dates that can be met by all parties Provide feedback on social services role in the plans for discharge Due to the high number of applications that we receive, only the most successful candidates will be contacted immediately.
